Mental Health Rehabilitation Service

The Mental Health Short Stay Rehabilitation Service accommodates 6 service users within walking distance (or a short bus journey) of Liverpool city centre.

Our aim is to provide short-term interventions, with the 24 hour support of experienced staff with knowledge of Mental Health and Learning Disabilities issues, to prepare service users to move to long-term appropriate accommodation. (6 – 18 months).

The facility enables service users to move into the community, allowing them to receive on-going rehabilitation around independent living skills, whilst we monitor their Mental Health.

Staff have close links with NHS staff and community support teams to allow the smooth transfer of information around a service user’s needs and risks.
Working with area teams such as the CLDT, CMHT, assertive outreach and crisis resolution teams, our people will work to a support plan to re-introduce the service user back into the community.

This is in line with the Care Programme Approach, or Effective Care co-coordinator process, which would be the vehicle to plan, deliver and review all care and support plans.

Target Service User Group

  • Service users with severe and enduring Mental Health issues, who are fit for discharge
  • Service users with a dual diagnosis learning disabilities/mental health issues
  • Service users with a Leaning Disability
  • Service users placed in inappropriate accommodation, requiring a period of rehabilitation, before moving into independent accommodation
  • Service users with a history of substance abuse

Moving-on Service

Once a service user is assessed as being able to move to independent accommodation, our staff facilitate a move to a supported scheme by addressing issues relating to housing benefit, purchasing of domestic items, application to social fund and the logistics of the relocation.

When these issues have been correctly dealt with we hand over to our partners in ‘area support services’.

The service user is at the centre of service design and delivery.  This means that we believe in offering a service enabling independent living that is person-centred to the service user.

We believe it is important to work with service users if we are to achieve an outcome-focused service that we can demonstrate to Commissioners.  Where appropriate that will include carers, advocators and other stakeholders.

At Falkner Square we provide the service on a ‘Recovery’ model.   Here services reflect the standards of relevant national policies and procedures, including the National Service Framework for Mental Health.

  • Service users have an individual room and share the communal kitchen, dining room, lounge and bathroom
