We provide ‘Supported Living Services’ for adults assessed as needing high levels of personal care and support living in residential services within the local community. These services social inclusion and independence for those with Mental Health issues and Learning Disabilities.
We do this by providing:
- Appropriate and assessed staffing levels
- Staff with the appropriate level of understanding, skills and experience to meet the needs of the residents.
- Individual person-centred plans which highlight individual care and support needs
- Specialist equipment to facilitate the necessary care and support
- Planned programmes of activities including holidays and weekend breaks
We work to enable people to live with a greater degree of independence by providing guidance on outcome focused activities such as budgeting, shopping, and other independent living skills as well as providing prompts with personal care, medication and dietary requirements.
As with all of our activities, we work in partnership with service users, Carers and stakeholders such as Care Managers, General Practitioners, Community Nurses and organisations who are similarly committed to providing independent living support.
We aim to provide a continuity of care which meets the changing needs of our service users, this requires that individual support is reviewed regularly.
Local Authorities
Our cost-effective and personalised support enables Local Authorities to meet statutory responsibilities through the implementation of effective outcome-focused support plans.
Service Users
The service user is at the centre of our work. We use person-centred thinking/approaches to achieve positive outcomes.
Carers
We listen to Carers. This enables us to provide the right support.
NHS Trusts
We work closely with relevant support and care teams within NHS Trusts to ensure that service users have access to and benefit from all of the assistance available to them.
